What you must do if you decide to change your current benefit option

If you want to change to one of the other LA Health benefit options for 2018, you can ask your broker to help, or download the Option change form below.

Please tell your employer or pension fund by 30 November 2017 if you want to make a change. Please make sure your employer or pension fund communicates the change to LA Health Medical Scheme so you get the correct benefits from 1 January 2018.
